Authorization to Release Medical Information Form

The Authorization to Release Medical Information form is to be completed only if you would like to grant permission to a family member/caregiver to speak with our office on your behalf.

If you are seeing our doctors for a cardiac surgical evaluation, please complete the top of the form prior to your appointment. Please note that our physicians perform the cardiac portion of a pre-operative assessment. We encourage patients to contact their primary care/family physician to obtain medical clearance and the necessary blood work for any upcoming surgery.
